Mediante la presente, yo autorizo a Common Wealth Charlotte a utilizar mi imagen, voz y testimonios (historia), captados a través de fotografías, videos o cualquier otro medio digital, en todas y cualquiera de sus publicaciones, incluyendo, pero no limitado a, materiales impresos, redes sociales, sitios web y otras plataformas digitales, sin ningún tipo de pago, compensación o remuneración.
Entiendo y acepto que dichas fotografías, videos y testimonios serán propiedad exclusiva de Common Wealth Charlotte y que no me serán devueltos.
Asimismo, autorizo a Common Wealth Charlotte a editar, modificar, copiar, reproducir, exhibir, publicar o distribuir este material para cualquier propósito lícito y legítimo. Renuncio expresamente a cualquier derecho de inspeccionar o aprobar el producto final en el que se utilice mi imagen, voz o testimonio. De igual manera, renuncio a cualquier derecho a regalías u otra compensación que pudiera derivarse del uso de los mismos.
En caso de que asista acompañado(a) de menores de edad, declaro que soy su padre, madre o tutor(a) legal, o que tengo la autoridad necesaria para otorgar este consentimiento en su nombre, y autorizo igualmente el uso de la imagen, voz y/o testimonios de dichos menores bajo los mismos términos establecidos en esta autorización.
Por medio de la presente, exonero y libero de toda responsabilidad, de manera permanente, a Common Wealth Charlotte, sus empleados, voluntarios, representantes y aliados, de cualquier reclamo, demanda o acción legal que yo, mis herederos, representantes legales, albaceas, administradores u otras personas que actúen en mi nombre o en nombre de mi herencia pudieran presentar en relación con esta autorización.
HE LEÍDO Y ENTIENDO ESTA AUTORIZACIÓN PARA EL USO DE FOTOGRAFÍAS, VIDEOS Y TESTIMONIOS. CERTIFICO QUE TENGO AL MENOS 18 AÑOS DE EDAD Y ACEPTO LOS TÉRMINOS AL COMPLETAR EL SIGUIENTE FORMULARIO.
Photo and Testimonial Release

By signing this form, I hereby authorize Common Wealth Charlotte to use my image, likeness, voice, and testimonials (story), captured through photographs, video, or any other digital media, in any and all of its publications, including but not limited to printed materials, websites, social media, and other digital platforms, without payment or other compensation.
I understand and agree that all photographs, videos, and testimonials shall be the exclusive property of Common Wealth Charlotte and will not be returned to me.
I further authorize Common Wealth Charlotte to edit, alter, copy, reproduce, display, publish, or distribute these photographs, videos, and testimonials for any lawful and legitimate purpose. I waive any right to inspect or approve the final product in which my likeness, voice, or testimonial may be used. I also waive any right to royalties or other compensation arising from or related to the use of my image.
If I attend accompanied by minor children, I certify that I am the parent or legal guardian of such minor(s), or that I otherwise have the legal authority to grant this consent on their behalf, and I hereby authorize the use of the image, likeness, voice, and/or testimonials of such minor(s) under the same terms and conditions set forth in this authorization.
I hereby release and forever discharge Common Wealth Charlotte, its employees, volunteers, representatives, and partners from any and all claims, demands, or causes of action that I, my heirs, legal representatives, executors, administrators, or any other person acting on my behalf or on behalf of my estate may have or may have in the future in connection with this authorization.
I HAVE READ AND UNDERSTAND THIS PHOTO, VIDEO, AND TESTIMONIAL RELEASE. I CERTIFY THAT I AM AT LEAST 18 YEARS OF AGE AND AGREE TO THESE TERMS BY COMPLETING THIS FORM.
